Antibiotic Consumption in Intensive Care Units in Some Hospitals in Sana’a City

AbstractBackground: Monitoring antibiotic consumption is crucial to tackling antimicrobial resistance. However, there is currently no system in Sana'a city for recording and reporting antibiotic consumption. This study is the first in Sana'a, Yemen to evaluate antibiotic use in ICUs using the Defined Daily Dose (DDD) and Days of Therapy (DOT) methods.
Objectives: To estimate antimicrobial consumption in intensive care units (ICUs) of hospitals in Sana'a city.
Methods: A retrospective study was conducted using ICU records from five major hospitals during 2020. Antimicrobial consumption data were mainly collected manually and analyzed using DDD and DOT per 1000 patient-days.
Results: The study included 1,970 patients. Total antibiotic consumption was 18,017.91 DDD per 1000 patients and 17,448.73 DOT per 1000 patient-days. Ceftriaxone, vancomycin, and meropenem were the most frequently consumed antibiotics by both DDD and DOT methods. Cephalosporins, carbapenems, and quinolones were the most consumed therapeutic groups. The WHO AWaRe "Watch" category accounted for the highest antibiotic consumption.
Conclusion: High consumption of ceftriaxone, vancomycin, and meropenem was observed in ICUs of the five hospitals. The findings emphasize the urgent need for an antimicrobial stewardship program to optimize antibiotic use. Future studies combining antimicrobial consumption with resistance monitoring are recommended.
