Assessment of Scrub Nurses' Knowledge and Practice Toward Sterile Techniques in Operation Rooms at Public Hospitals in Sana'a City, Yemen

Background: Maintaining sterile technique in operating rooms is essential to prevent contamination and reduce the risk of surgical site infections. Scrub nurses are central to establishing and maintaining the sterile field; however, gaps in knowledge and inconsistent adherence to aseptic practices can compromise patient safety. Objective: To assess the level of knowledge and practice of scrub nurses toward sterile techniques in operating rooms at public hospitals in Sana’a City, Yemen. Methods: A descriptive cross- sectional study was conducted in the operating rooms of Al-Thawra, Al-Jumhuri, and Al- Kuwait University hospitals. Using convenience sampling, 240 eligible scrub nurses were recruited across shifts. Data were collected via a structured, self-administered questionnaire adapted from validated tools: demographics/work profile; knowledge; practice. Descriptive and inferential statistics, including Pearson correlation, were applied. Results: Participants were predominantly ≤35 years (77.5%) and female (55.0%). Most held a higher diploma (42.9%) or bachelor’s degree (38.3%); 48.8% had <4 years’ OR experience, and 54.6% had not received sterilization training. Overall, 55.4% had insufficient knowledge and 52.9% reported inadequate practice. Knowledge was high for general sterile-field rules but low for critical intraoperative decision-making (e.g., handling sterile water requests, responding to glove perforation, and safe movement near sterile fields). Practice showed moderate, inconsistent adherence, with notable deficits in environmental control and waste segregation. Knowledge was positively related to practice (r = 0.485, p < 0.001). Knowledge and practice differed significantly by age, sex, marital status, hospital, education, experience, and training, while daily working hours were not significantly associated. Conclusion: Scrub nurses demonstrated suboptimal knowledge and practice of sterile technique, with pronounced gaps in high-risk intraoperative actions and selected infection-control behaviors. Recommendations: Regular competency-based education (including simulation), routine auditing with feedback, and standardized OR protocols—particularly for intraoperative contamination events and waste management—are recommended, prioritizing staff without prior training.
