الملخص

الملخص البحثي

​الخلفية والأهداف:

تناولت هذه الدراسة المقطعية الوصفية المستقبليّة تقييم مدى انتشار نقص الوزن التغذوي وعوامل الخطورة المرتبطة به بين الأطفال الذين تتراوح أعمارهم بين 6 أشهر و6 سنوات المولودين للموظفات في المستشفيات الحكومية بمدينة صنعاء، اليمن. انطلقت الدراسة من فرضية مفادها أن الكوادر الصحية النسائية يمتلكن وعياً وتثقيفاً أعلى بأسباب ومخاطر سوء التغذية. وتم تقييم نمو الأطفال باستخدام مخطط النمو للوزن بالنسبة للعمر الصادر عن مراكز السيطرة على الأمراض والوقاية منها (CDC).

​المنهجية والعيّنة:

شملت الدراسة 278 موظفة في المستشفيات (116 ممرضة، 49 فنيّة، 49 من الكادر المخبري، 39 طبيبة عامة، و27 أخصائية). أُستُبعد الأطفال المصابون بتشوهات خلقية أو أمراض مزمنة. جُمِعت البيانات عبر استبيانات ومقابلات منظمة، وتم أخذ قياسات الوزن الميدانية المباشرة لـ 45% من الأطفال، بينما استُند في بقية الحالات إلى إفادات الأمهات.

​النتائج الرئيسية:

​بلغت نسبة انتشار نقص الوزن التغذوي (الوزن بالنسبة للعمر أقل من المئين الخامس 5th percentile) بين أطفال الكادر الصحي النسائي 41.5%.

​أبرز عوامل الخطورة المرتبطة:

​عمل الأم في مهنة التمريض (سجلت أعلى نسبة انتشار بـ 49%).

​عمر الطفل ما بين سنة واحدة و3 سنوات.

​ترك الأطفال في المنزل دون وجود مربية أو ربة منزل متفرغة لرعايتهم.

​مناوبات عمل الأم التي تتجاوز 12 ساعة يومياً.

​وجود أكثر من طفلين تحت سن السادسة في نفس الأسرة.

​انخفاض المستوى الاقتصادي والاجتماعي (مثل عدم ثبات دخل الأب، أو السكن بالإيجار).

​وجود تاريخ للولادة المبكرة (أقل من 37 أسبوعاً)، أو كون الطفل ذكراً، أو انخفاض الوزن عند الولادة.

​الاستنتاج:

بغض النظر عن المستوى التعليمي أو التوعية الصحية للأم، فإن ساعات العمل الطويلة والغياب عن المنزل ينعكسان سلباً على نمو الطفل في حال غياب الرعاية والاطعام المناسبين أثناء غياب الأم، أو عند الاقتران بالظروف الاقتصادية الصعبة.

​أهم التوصيات:

​توعية الأمهات العاملات بتحضير وجبات مغذية ومخصصة للطفل قبل الذهاب لمناوبات العمل.

​تشجيع تنظيم الأسرة لتجنب وجود أكثر من طفلين دون سن السادسة، خاصة للأمهات اللاتي يعملن لفترات طويلة (أكثر من 12 ساعة/يوم).

​ضمان عدم ترك الأطفال الصغار (أقل من 3 سنوات) دون رعاية واعية، وإرشادات لإنشاء برامج وطنية للمتابعة الدورية لنمو الأطفال.
