الملخص

الخلفية: تُعد أورام الدماغ والجمجمة من الأسباب الرئيسية للمراضة العصبية. وفي البيئات محدودة الموارد مثل اليمن، حيث غالباً ما يتعذر إجراء التصوير بالرنين المغناطيسي (MRI)، يظل التصوير المقطعي المحوسب (CT) الأداة التشخيصية الأساسية. ومع ذلك، تفتقر الأدبيات الطبية إلى بيانات منهجية حول خصائص هذه الأورام في التصوير المقطعي لدى السكان اليمنيين. تهدف هذه الدراسة إلى تقييم الخصائص الديموغرافية وسمات التصوير المقطعي المحوسب (بمعاييره المتعددة) لأورام الدماغ والجمجمة في المركز الوطني للأورام بصنعاء، اليمن.

الطرق: أُجريت دراسة وصفية مقطعية في الفترة من أكتوبر 2025 إلى فبراير 2026، شملت 52 مريضاً متتالياً (تتراوح أعمارهم بين 2 و75 عاماً) ممن تأكدت إصابتهم بأورام في الدماغ أو الجمجمة وخضعوا لتصوير مقطعي محوسب مع استخدام مادة تباين (صبغة). تم تحليل البيانات الديموغرافية وخصائص الأورام وسمات التصوير المقطعي (الكثافة، والتعزيز بالصبغة، والنخر، والتكلس، والوذمة، وتأثير الكتلة) باستخدام برنامج SPSS الإصدار 23.0، مع اعتبار مستوى الدلالة الإحصائية p < 0.05.

النتائج: شكلت الإناث الغالبية (65.4%). وكانت الأورام النقيلية (الثانوية) هي الأكثر شيوعاً (46.2%)، تليها أورام الجهاز العصبي المركزي الأولية (40.4%)؛ حيث كان سرطان الثدي هو المصدر الأولي الرئيسي (17.3%). وكان الدماغ (المخ) هو الموقع الأكثر شيوعاً للأورام (48.1%). ولوحظ أن الآفات المتعددة كانت أكثر شيوعاً بشكل دال إحصائياً في الأورام النقيلية (54.2%) مقارنة بالأورام الأولية (28.6%) (p=0.002). وشملت الارتباطات الهامة: ورم المستقتمات (paraganglioma) مع الكثافة المماثلة (iso-density) (r=0.68) والتكلس (r=0.82)؛ وأورام الحجاج (orbital tumors) مع الكثافة المماثلة (r=0.72)؛ والأورام النقيلية مع النخر (r=0.45). كما تم رصد أنماط رئيسية للتزامن بين السمات، مثل التعزيز القوي بالصبغة مع النخر في الأورام النقيلية (p=0.001)، والكثافة العالية (hyperdensity) مع التكلس في الأورام السحائية (meningiomas) (p=0.004).

الخلاصة: يُعد التصوير المقطعي المحوسب وسيلة فعالة لتوصيف أورام الدماغ والجمجمة في البيئات محدودة الموارد. وتوفر أنماط سمات التصوير المقطعي المميزة وارتباطاتها الإحصائية أساساً لخوارزمية تشخيصية أولية، مما يعزز التشخيص المبكر واتخاذ القرارات السريرية في الحالات التي يتعذر فيها إجراء التصوير المتقدم أو الفحص النسيجي المرضي.
